Billing, Insurance,  and Payment

We accept most major insurance plans!
Please call our office at (615) 472-1550 to ask our staff if we accept your insurance. We do not accept Medicaid or TennCare.

If your insurance plan requires you to pay a co-payment, co-insurance, and/or a deductible, you will need to pay at the time of your visit. For your convenience we accept cash, checks and major credit cards.


Self-Pay Pricing
For patients without insurance, we have fair and transparent pricing so that you can know before your visit what to expect to pay for your visit.

To check in prior to your visit, you may use our online check-in form.

You can also pre-register by downloading and printing the forms below. Simply fill them out and bring them with you to your visit. Completing the forms prior to your visit will help speed up the new patient registration process.
